This is an order form for custom Discraft Ultra-Star™ Sportdiscs™. When you send this form, you will be taken to another web page to upload your artwork if it is ready. Once we receive your order, we will fax or email you a quote within 2-3 business days.

This order form is for one custom design (your artwork) which you would like printed on the Discraft Ultra-Star™. If you have more than one design, a separate order form must be completed for each design.

The minimum order for custom stamping is 100 discs per design.

UPA 20 PERCENT DISCOUNT ELIGIBILITY:
To qualify for a 20% UPA discount on custom Ultra-Stars, you must be a current UPA member, and your order must be for a tournament, club/league, or team. If the bill or ship to person is a UPA member, we will need his or her name and UPA number before a discound quote can be given:
United States residents only: The bill or ship to person must have a current UPA number at the time your order form is submitted to receive the UPA member discount.

The minimum order for a 1 or 2 color design is 100 discs. There is a minimum of 200 discs for a 3 color design. Please allow 4 to 5 weeks for delivery. There is an extra charge for all rush orders, call 248-624-2250 for details.
There are a certain number of disc and hotstamp colors included in your order based on the quantity of discs you are ordering and whether yours is a one color or a two color design. See chart below. A $20 fee will be charged for each additional disc or print color beyond these allowances.
100 - 199 Piece Order
1 color design You may choose up to two disc colors and two different hot stamp (print) colors.
2 color design You may choose up to two disc colors and two different hot stamp (print) colors. The same two hot stamp colors will be printed on all discs. Maximum of two combinations.
200-299 Piece Order
1 color design You may choose up to three disc colors and three different hot stamp (print) colors.
2 or 3 color design You may choose up to three disc colors and four different hot stamp (print) colors. Maximum of three combinations.
300 or More Piece Order
1 color design You may choose up to four disc colors and four different hot stamp (print) colors.
2 or 3 color design You may choose up to four disc colors and four different hot stamp (print) colors.

There is a maximum of four combinations included.

Misprints of your design are available at the time your order ships and are $3.00 each. There are generally 0 to 10 misprints of 100 to 200 and prepaid.
